General

Change of Records Form

Members may use the Change of Records Form to make the following changes to their UBC Faculty Pension Plan account:

  • Address changes (for Deferred and Retired members only; Active member address changes must be made through Workday)
  • Name changes (for Deferred and Retired members only; Active member address changes must be made through Workday)
  • Marital / relationship status declaration
  • Beneficiary changes

By completing a Change of Records Form, all previous beneficiary designations will be revoked. Therefore, all sections relating to beneficiary designations must be completed.

To confirm the change, your beneficiary information is available by signing into mysunlife.ca/ubcfpp, then under Investments, select Manage plan > my plan > Make a change > Manage beneficiaries. Your quarterly statement of account also confirms your beneficiary information.

Third Party Authorization Form

Members can use the Third Party Authorization Form to provide authorization to a Third Party to have access to information about their UBC Faculty Pension Plan account and/or act on their behalf to make various account changes/transactions.

Beneficiary Designation & Authorization Form (for New Faculty Pension Plan Enrolments Only) 

Members who recently enrolled in the UBC Faculty Pension Plan may use the Beneficiary Designation & Authorization Form to make their beneficiary designation. This designation is separate and apart from your beneficiary designation under the UBC Supplemental Arrangement (if applicable). Members may make changes to their existing beneficiary using a Change of Records Form.

Form 4 – Spouse’s Waiver of Beneficiary Right to Benefits in a Pension Plan, Locked-In Retirement Account, Life Income Fund or Annuity Before Payments Start

In the event that a member with a spouse wishes to designate someone other than their spouse as the primary beneficiary, the spouse must complete Form 4 – Spouse’s Waiver of Beneficiary Right to Benefits in a Pension Plan, Locked-In Retirement Account, Life Income Fund or Annuity Before Pension or Annuity Payments Start. Application for Direct Transfer of Assets into the Faculty Pension Plan

Active members can easily consolidate registered plans online through their member account. Sign in at mysunlife.ca/ubcfpp, then under Investments, select Manage plan > my plan > Put money in > Transfer registered plans to the UBC FPP and follow the prompts.

Deferred and Retired members can transfer external registered assets from a financial institution by using the Application for Direct Transfer of Assets Form.

Termination Benefits

The following forms relate to the transfer of funds by a member upon the end of their appointment. See Leaving UBC for more information.

Request to Withdraw Cash from Non Locked-In Funds

If a member wishes to withdraw cash from the pension plan, they can do so by contacting the Sun Life Client Care Centre at 1-844-822-3131 (1-844-UBC-3131). There is no form for requesting cash withdrawals.

Direct Transfer of a Single Amount (T2151)

The Direct Transfer of a Single Amount Form (T2151) must be completed by a member who wishes to transfer their funds out of the UBC Faculty Pension Plan to another registered plan. Locked-in FPP contributions must be transferred to an account that is registered under the BC Pension Benefits Standards Act. Any locked-in funds that were transferred-in to the Plan will remain under the originating pension legislation. Please submit this form to:

Form 3 – Spouse’s Consent to a Transfer to a Life Income Fund or Establishment of a Life Income Type Benefits Account

If any portion of a member’s account is used for a Life Income Fund (LIF) or LIF-Type Payments, the member’s spouse must complete a Form 3 – Spouse’s Consent to a Transfer to a Life Income Fund or Establishment of a Life Income Type Benefits Account Form. Form NR73, Determination of Residency Status (leaving Canada)

Members with locked-in funds may apply to have the lock-in restriction removed from their funds if they have not resided in Canada for a minimum of two years. A member must first apply to the Canada Revenue Agency (CRA) by completing and submitting a Form NR73, Determination of Residency Status (leaving Canada) to the CRA.

Upon receipt of the form, the CRA will provide a written opinion on your residency status. See Living Outside Canada for instructions to remove the lock-in restriction.

Instructions for Form 1 – Spouse’s Waiver to Permit Benefits in a Pension Plan, Locked-In Retirement Account or Life Income Fund to be Unlocked

In the event that a member has obtained non-residency status from CRA and wishes to unlock their locked-in funds, the member’s spouse must waive their entitlement to receive survivor’s benefits by completing the Form 1 – Spouse’s Waiver to Permit Benefits in a Pension Plan, Locked-In Retirement Account or Life Income Fund to be Unlocked. Form 2 – Spouse’s Waiver of 60% Lifetime Survivor Benefit and/or Beneficiary Rights From a Pension Plan or Annuity After Payments Start

In the event that a member wishes to elect less than a 60% survivor benefit, the member’s spouse must waive their entitlement to the minimum 60% death benefit by completing Form 2 – Spouse’s Waiver of 60% Lifetime Survivor Benefit and/or Beneficiary Rights Form From a Pension Plan or Annuity After Payments Start. Retirement

A member may elect to retire under the Plan as early as the first of the month following their 55th birthday or the first of any succeeding month. The following forms relate to the commencement of retirement benefits under the Plan. See Retiring for more information on retirement.

 

Retiree Checklist

Members may use the Retiree Checklist to assist them with completing their application to commence a UBC Faculty Pension Plan VPLA, RRIF-Type or LIF-Type Payment option.

Variable Payment Life Annuity Application Form

The Variable Payment Life Annuity Application Form is completed by a member who wishes to commence a Variable Payment Life Annuity under the UBC Faculty Pension Plan.

LIF-Type Payments Application Form

The LIF-Type Payments Application Form is completed by a member who wishes to commence LIF-Type Payments under the UBC Faculty Pension Plan.

RRIF-Type Payments Application Form

The RRIF-Type Payments Application Form is completed by a member who wishes to commence RRIF-Type Payments under the UBC Faculty Pension Plan.

Spousal Status Declaration Form

If you do not have a spouse, please complete a Spousal Status Declaration Form.

Personal Tax Credit Return – Federal TD1

A Personal Tax Credit Return – Federal TD1 Form must be completed for the federal government.

Personal Tax Credit Return – Provincial TD1BC

A Personal Tax Credit Return – Provincial TD1BC Form must be completed for your provincial government.

Form NR301, Declaration of Eligibility for Benefits (Reduced Tax) Under a Tax Treaty for a Non-Resident Person

If you are a non-resident of Canada, a Form NR301, Declaration of Eligibility for Benefits (Reduced Tax) Under a Tax Treaty for a Non-Resident Person must be completed.

Direct Deposit Enrolment Form

To enrol for direct deposit to your account at a financial institution or to update your banking information, please complete the Direct Deposit Enrolment Form.

Request for Benefit Payment by Wire

If you do not have a bank account at a Canadian financial institution, you may elect to have your payments wired to your financial institution. Enrol or update your banking information by completing the Request for Benefit Payment by Wire Form. Please note: your receiving financial institution may charge a fee to accept wire transfers.

Direct Transfer of a Single Amount (T2151)

Marriage Breakdown

The following forms relate to transfers out of the Plan upon marriage breakdown. See Marriage, Common Law, Separation, & Divorce for more information regarding UBC Faculty Pension funds upon marriage breakdown.

Form P1 – Claim and Request of Information and Notice

Form P1 – Claim and Request for Information and Notice is completed by the spouse or former spouse of a member who would like to claim an interest in the member’s/annuitant’s benefits.

Form P3 – Request for Transfer from Defined Contribution Account

Form P3 – Request for Transfer from Defined Contribution Account is completed by the spouse or former spouse of a member who would like to transfer their pension entitlement to their RRSP or RPP.

Form P9 – Agreement to Have Benefits Divided

Form P9 – Agreement to Have Benefits Divided can be used to satisfy the requirement for an agreement, under Part 6 of the Family Law Act. DO NOT USE THIS FORM if you already have a written agreement, or an order, dividing the benefits.

Direct Transfer of a Single Amount (T2151)

The Direct Transfer of a Single Amount (T2151) form must be completed by a former spouse for each transfer to an RRSP, RRIF, RPP or LIF.

 


Supplemental Arrangement

The following forms are for members of the Supplemental Arrangement.

Application for Retirement/Termination Benefits – Supplemental Arrangement Form

To receive benefits following termination of employment, complete the Application for retirement/termination benefits – Supplemental Arrangement form and Direct deposit enrolment form (Canadian banks only) and submit the forms to the Pension Administration Office.

If you do not have a bank account at a Canadian financial institution, you may elect to have your payments wired to your financial institution. Enrol or update your banking information by completing the Request for benefit payment by wire form. Please note: Your receiving financial institution may charge a fee to accept wire transfers.

To confirm the change, your beneficiary information is available by signing into mysunlife.ca/ubcfpp. Under Investments, select Manage Plan > my Plan > Check on Account > View accounts > Click on your account number beside UBC Supplemental Arrangement > Make a change > Manage beneficiaries.  Your quarterly statement of account also confirms your beneficiary information.
